You need to find out what's causing your bad breath! It could be a dry mouth, or it could be something serious. But the fact that it comes back in the afternoon tells me it might be just a bad habit or chronic condition.

Try drinking a lot of water throughout the day, if you don't already. I can't emphasize this enough!

Avoid coffee and dairy products, especially milk; they're not very healthy and they're terrible for your breath. USE FLOSS - this will get rid of smelly food between your teeth. Brush your tongue too. I carry cinnamon candies with me to get rid of coffee breath - I think it smells better on your breath than mints. If there's nothing else around, try gargling with plain water.

Bad breath can come from your sinuses too. Do you have constant congestion or sinus infections? Avoiding dairy products, soy and fruit juice (especially orange juice) may help. Drinking lots of plain water will definitely help. Taking Vitamin C helps ward off sinus infections. Seeing an allergist might help too.
References :
I worry about it too!

Drink plenty of water. Stay away from sugar such as gum, mints, soda. It actually promotes the growth of the bacteria living on your tongue. The bacteria can also cause tonsil stones which are white or yellow globs that sit in the small pockets of your tonsils. They smell horrible. The only way to really get rid of bad breath is by getting rid of the bacteria. Here is a free ebook that tells all from causes to cures. http://www.yourbadbreathremedy.com
References :
You should be brushing after every meal, roughly three times per day for at least two minutes at a time. You should be using floss after every meal, and use mouthwash and a tongue scraper at least twice each day. Pay attention to what kinds of foods that you are eating. Also, do you have problems with dry mouth? Dry mouth and post nasal drip can cause bad breath, so keep hydrated. Have regular dental checkups (roughly once every six months) to ensure that you do not have cavities or gum disease. Also, you should go to the doctor if it is a concern for you, since Underlying health conditions like diabetes can cause bad breath.

You might want to avoid gum and mints that contain sugar because sugar actually FEEDS the bad breath-causing bacteria!

You can also try natural cures such as baking soda, green cardamom, parsley/mint garnishes, brussel sprouts, and green tea. These help deodorize the halitosis.
References :
http://therabreath.com

If bad breath is caused by dry mouth simply eating a citrus fruit that is high in citric acid (such as an orange, lemon, or grapefruit) can help improve your breath significantly. The acid in such fruits stimulates the saliva, which helps suppress some of the odor-causing bacteria. At the same time, the tangy taste of such fruits leaves the mouth smelling fresh.
Although only a short-term fix, mouthwash or mints can temporarily mask the bad breath caused by most foods (only time can really cure garlic or raw onion breath). The best solutions are those that help get rid of bacteria in the mouth:

Regularly floss your teeth to eliminate debris around the gumline.
Give your teeth and tongue a good brushing after meals (bacteria is often embedded around the taste buds and toward the back of the tongue).
Consider a tongue scraper. Does the job better than a toothbrush.
Drink lots of water to keep your mouth moist.
Chew sugarless gum. Mint and cinnamon flavors will temporarily mask bad odor...plus, the stickiness helps remove food particles and debris on your teeth. Chewing gum also encourages the flow of saliva, which keeps your mouth moist.
Chew on a sprig of parsley. Parsley's chlorophyll is thought to break down bacteria in the mouth and act as a mouth freshener.
Try fennel seeds. Chewing on about ¼-½ teaspoon can help clear a bad taste from your mouth.

If you clean up your diet and still have dragon breath, speak with your doctor to rule out any underlying illnesses. Many medical conditions can contribute to bad breath, including periodontal disease, chronic sinusitis, postnasal drip, respiratory tract infections, xerostomia (dry mouth), gastrointestinal issues, diabetes, or liver or kidney disease.
References :
Same problem.
Apparently brushing a lot and listerine isn't working.
I found a way,
TRIDENT CHEWING GUM
It rocks Man.
After spitting out chewing gum, the bad smell goes for 1-3 hours.
Super Good.
